Report to show ADSAccount Disable Date

I've been working on building a report to show the exact date the ADS Resource flag is checked.  I can see that its easily viewable in the Process History on an ADS resource but am having a difficult time translating this either into a SQL query that would return column like SamAccountName, EmployeeID, and the AD Account Disable Date or a Report through Report Editor.  Has anyone seen where this particular attribute is stored within the Q1IM database or know the corret way to show this via report editor?

This is the format I'm looking for.  Does anyone know or have an example of how to pull the exact time stamp that the AccountDisabled@ADSAccount flag is changed either via a query or Report?
